Haas identifies P/N 04-0163 / BT40-SM.750-40 as a BT40 shell mill holder with a 3/4" pilot diameter and 40 mm gauge length.
The Accusize 2" cutter specification matches a 2" cutting diameter, 3/4" arbor hole and five APKT1604 insert positions. Accusize specifies this style for face milling, shoulder milling, side milling, slotting, ramping and related operations.

Haas BT40 shell mill holder
BT40 M16 retention knob/pull stud
Carbide inserts currently installed, exactly as pictured
Condition: Used industrial tooling. Shows normal cosmetic wear, machining marks, discoloration and light surface oxidation from previous shop use/storage. Cutting inserts show prior use. I have not precision-tested the assembly for runout or cutting performance.
